chore: bump dist/index.js to v2.5.9 to force dashboard reload 

- add version comment to dist/index.js so dashboard static cache busts
- manifest 2.5.9, plugin.yaml 2.5.9
- fixes stale dist/index.js at 2.5.4 while manifest was at 2.5.8
